After having reached the potential winners and having verbally confirmed, with witnesses, their eligibility for the 5/30 Health Challenge prizes, the organizers will supply, individually to all the potential winners, a written declaration attesting to the respect of the conditions set out in the present rules, as well as a waiver.

If the deadline is not respected, the potential winner will be disqualified. A parent or guardian must sign the declaration of a potential winner who is a minor.
