Registration
1. Registration for the 5/30 Health Challenge contest takes place from December 19, 2007 at noon [EST] to February 29, 2008 at 11:59 PM [EST] (the “contest duration”).2. All individuals interested in participating in the 5/30 Health Challenge contest must, before February 29, 2008 at 11:59 PM [EST], fill out and send A SINGLE REGISTRATION FORM for the 5/30 Health Challenge contest: either a paper registration form, duly filled out, signed, and sent by mail in an envelope with sufficient postage, or by fax, or by using the on-line registration form on the website of the 5/30 Health Challenge. All registration forms must be sent by February 29, 2008, 11:59 PM [EST] at the latest. The postmark will be proof of the date and time of the mailing of the paper form. For forms sent by fax or Internet, the date and time of reception will serve as proof. If, due to computer problems, the registrations cannot be adequately integrated into the Challenge website by February 29, 2008, the organizers may decide to accept registrations during a supplementary period. If this is the case, a special note to this effect will appear on the home page of the 5/30 Health Challenge website.
3. The registration form must be sent by one of the following means:

4. Each participant will receive a registration confirmation (immediately on the website, if registering on-line, and by mail if registering using the paper form).
5. In submitting the registration form for the 5/30 Health Challenge on-line, by mail, or by fax, candidates agree to actively participate in the 5/30 Health Challenge by attaining or maintaining its “5” and “30” objectives and to take up the 5/30 Health Challenge in their own way, according to their reality and preferences, in order to improve their health.
6. It is possible to register as a “family” (child(ren), father, mother) and be eligible for the “5/30 Desjardins Family” prize. All registered families will automatically be eligible for the “5/30 Desjardins Family” prize. A family is described as follows: any adult 18 or older, who is a parent of at least one child 17 or younger, living at the same address. To register a family, an adult family member (the “family representative”) must, before February 29, 2008 at 11:59 PM [EST], fill out and send A SINGLE REGISTRATION FORM for the 5/30 Health Challenge contest, available at www.530healthchallenge.ca.
7. In submitting the on-line registration form for the 5/30 Health Challenge, the “family representative” (an adult family member) must confirm the first and last name of each family member and indicate the family’s contact information (including the telephone number where they can be reached). The family will immediately receive a registration confirmation on the website. The “family representative” must also briefly describe and explain the Challenge objectives to the family members and encourage them to achieve them.
8. It is also possible to form a group of 2 to 10 people to participate in the 5/30 Health Challenge as a team (work colleagues, friends, extended family, etc.). To create a team, or to join an existing team, participants must visit the To Register section at www.530healthchallenge.ca and fill out the registration form in the As a team (colleagues, friends, extended family, etc.) area. The participant who is the team “initiator” will be responsible for the created 5/30 team and can invite up to 9 other members to join. A participant can be a member of only one team.
9. In submitting the on-line registration form for the 5/30 Health Challenge,
“family representatives” or team “initiators” declare that they have read and
understood the rules of the 5/30 Health Challenge, satisfy them, are committed to
respecting them and communicate the information to the registered family or team members.
